This 12-week
comprehensive lifestyle management program provides
our uninsured and
underinsured clients with the necessary tools to make
lasting lifestyle
changes that improve their overall health, reduce
dependency on medications
and reduce unnecessary medical expenses.
Each client is required to make a commitment to the 12-week program.
This includes exercise three times a week in our wellness center, attending
educational lectures, meeting with a dietitian for nutritional counseling
and
attending smoking cessation classes if indicated. In exchange for
their
commitment to their health, we will provide limited monthly prescription
assistance.
When people invest in their health they change their role from a passive
state to
one of an active individual, taking charge of their well-being.
Since the beginning
of this program in July of 2003, we have been able
to see a transformation in
people's health. Changes ranging from weight
loss, reduction in blood pressures,
improved diabetic control, reduced
dependency on certain medications and
improved level of physical and emotional
well-being are some of the most
prevalent changes noted.
